CUNA’s newly formed Working Group on Supervisory Issues, in coordination with your State Credit Union League, is analyzing examination and supervisory issues raised recently by credit unions around the country. The Working Group is developing a report that will address these concerns and provide guidance to credit unions in dealing with their examiner and other agency staff when disputes over significant issues arise. It will also address the role and legal obligations of examiners and make recommendations to improve the supervisory process. In addition to preparing a report, the Working Group is establishing this form as a tracking mechanism to gather information on exam related issues on an on-going basis.


This form asks you to provide detailed information on your most recent examination. The information you provide will be used by CUNA’s Economics and Research Department to analyze trends in these areas. The information on these trends will also be discussed in the upcoming report, and to update the trends after the report is published.

The information you provide to us will be treated as strictly confidential and only reported as summary information without identifying individual credit unions. There is an optional section at the end of the form that requests information that allows us to cross-reference your report with your credit union’s statistical profile, and we encourage you to complete it with the knowledge that the identity of your credit union will be restricted to CUNA and your league. Alternatively, you can submit this form by omitting the optional information, but this omission will make the report less useful to us.
